Who am I?
I am a Geriatric and Palliative Care Nurse Practitioner with a decade of experience as a volunteer, nurse, and advanced practice provider in hospice, palliative care, and patient advocacy. I live with my husband and a rotation of foster dogs in Manhattan, New York. When I’m not talking about Advance Care Planning, you can find me dancing or listening to music, watching a Phillies baseball game, or on a nature walk with family or friends (ideally in Carpinteria, California - where I’m from!).

What do the letters behind my name mean?
In August of 2024, I completed my Doctorate in Nursing Practice (DNP) degree from New York University (NYU). My dissertation project created an efficacious, evidence-based Advance Care Planning program for LGBTQ+ Older Adults at SAGE. I also completed my Master’s in Nursing Science (MSN) at NYU, specializing in Adult-Gerontologic Primary Care (AGPCNP-BC) and Hospice and Palliative Care (ACHPN). In this Threshold Planning LLC role, I am not using any of my nursing licenses or certifications and I am only acting as an Advance Care Planning Coach, not as a medical professional.
What is my professional experience?
While getting my advanced nursing degrees, I worked on an inpatient oncology unit as a Registered Nurse (RN) at Memorial Sloan Kettering Cancer Center (MSKCC) after completing my Bachelors in Nursing from University of Pennsylvania. While at MSKCC, I was deeply involved with a number of committees coordinating our institutional approach to geriatric resources, supportive care services, end of life care, environmental sustainability, and LGBTQ+ health. During my time there, I received the Sherry Beilinson Most Vulnerable Patient Care award and the Hospice Palliative Nurses Foundation Emerging Leader award. I also worked as an RN for Calvary Hospice in the inpatient, nursing home, and home settings.
Now I work as a Palliative Care Nurse Practitioner with Carelon Health. I visit people who have advanced illness and aging challenges in their homes and apartments in Manhattan, Brooklyn, and the Bronx. In that role, I support my patients with advance care planning, symptom management, and care coordination within healthcare systems and hospice transitions to reduce the burden of their health issues and need for hospitalizations.
